SPRINT long-term stroke-prevention follow-up published

A peer-reviewed Blood report found no statistically significant difference in recurrent stroke or all-cause mortality by initial hydroxyurea dose assignment over 4.9 years. The abstract gives no group counts or effect estimates, so it does not establish equivalent dosing.

STATUS CHANGED

City of Hope haploidentical transplant pilot withdrawn

The US phase 1 registry now says withdrawn before enrolment, with zero actual participants and no treatment outcomes. This does not change access to established transplant programmes.

Previous
An archived registry mirror last updated 8 May 2026 listed the study as active and recruiting.

CURRENT STATE

The situation now

Plain-language conclusions first. Every claim is source-backed, dated and reviewed at least every 30 days.

UK ACCESS / CONFIRMED

Casgevy is available through managed NHS access in England, not as an unrestricted treatment.

NICE recommends Casgevy for a defined group aged 12 and over with severe sickle cell disease, recurrent vaso-occlusive crises and no matched related stem-cell donor. Eligibility is assessed through specialist services. [UK-NICE-001][UK-NHS-001]

The process is intensive: a person's own blood stem cells are collected, edited outside the body, and returned after conditioning chemotherapy. Long-term outcomes continue to be monitored. [EU-EMA-001][UK-NICE-002]

WATCHTRIAL RESULTS / REGULATORY REVIEW

Casgevy trial results are public; EMA review is still to confirm

ClinicalTrials.gov first posted final CLIMB-121 results on 16 September: 42 of 46 treated participants (91.3%) met the trial's 12-month severe-crisis-free endpoint. This single-arm result is not a lifetime cure rate and follows intensive conditioning chemotherapy.

Vertex also submitted final CLIMB-121 and interim long-term follow-up evidence to EMA for a product-information update. The public registry results do not by themselves confirm EMA adoption or change NICE managed access in England.

SYSTEMENGLAND / IN USE

Genomic blood matching has moved into national use

Patients in England are now receiving blood selected with detailed genomic blood-group information. The national programme is being rolled out to around 17,000 people with sickle cell disorder.

More precise matching is intended to reduce antibody formation and harmful transfusion reactions. The announcement confirms implementation, but does not yet provide national outcome data showing how much those complications have fallen.

RESEARCH ANGLES

Routes towards sickle cell treatment

Three gene-based strategies, each with two possible delivery paths. A working research map, not a list of available treatments.

SPECULATIVE FRAMEWORK

The numbers show Dan's monitoring order, not likelihood of success or clinical priority. This map does not cover established medicines, transfusion or supportive care. "With chemotherapy" means marrow-conditioning treatment before a stem-cell transplant, not chemotherapy used to treat sickle cell directly. No route here has demonstrated a durable sickle-cell cure in people without conditioning chemotherapy. Some proposed alternatives still use other drugs or selection steps.

ROUTE 01

Restore adult haemoglobin

Correct HBB so blood-forming cells produce HbA rather than HbS.

ASSUMPTION TO TESTCan the sickle mutation be corrected durably in enough long-term stem cells?
WITHOUT CONDITIONING CHEMOTHERAPY PRECLINICAL HYPOTHESIS

Edit HBB inside the body, without collecting cells or clearing marrow first.

LIMITTessera's non-human-primate result is company-reported. It is not a human sickle-cell outcome, and a chemo-free clinical pathway has not been demonstrated.

WITH CONDITIONING CHEMOTHERAPY EARLY CLINICAL PATHWAY

Correct HBB in collected stem cells, then reinfuse them after marrow-conditioning chemotherapy.

LIMITCRISPR_SCD001 has a recruiting trial, but no posted human treatment results. Its busulfan conditioning is substantial and it is not a UK treatment option.

NEXT EVIDENCE GATEHuman evidence for durable HBB correction, safety and clinical benefit without conditioning.

ROUTE 02

Convert HbS to HbG-Makassar

Change the sickle variant into a naturally occurring, non-sickling haemoglobin variant. This is not restoration of normal HbA.

ASSUMPTION TO TESTCan precise HbG-Makassar conversion persist in long-term stem cells and prevent sickling?
WITHOUT CONDITIONING CHEMOTHERAPY PRECLINICAL HYPOTHESIS

Deliver a base editor to stem cells inside the body, aiming to convert HbS to HbG-Makassar without marrow-conditioning chemotherapy.

MATCHED EVIDENCE
FOUNDATION
Breda et al. in-vivo stem-cell delivery

LIMITBreda et al. showed in-vivo stem-cell delivery in mice and HbG-Makassar conversion in donor cells ex vivo, not the combined approach. A separate non-genotoxic conditioning experiment does not prove this route works in people.

WITH CONDITIONING CHEMOTHERAPY PRECLINICAL TRANSPLANT MODEL

Base-edit collected stem cells to make HbG-Makassar, then transplant them into conditioned marrow.

MATCHED EVIDENCE
FOUNDATION
Newby et al. HbG-Makassar mouse work

LIMITThe published rescue evidence is from cells and mice, not treated people. Mouse transplantation does not define a human chemotherapy protocol. This is not an approved or established clinical route.

NEXT EVIDENCE GATEIndependent replication, durable long-term stem-cell editing and a human trial with meaningful sickle-cell outcomes.

ROUTE 03

Raise fetal haemoglobin

Reactivate HbF so it reduces the tendency of HbS-containing red cells to sickle. HbS itself remains.

ASSUMPTION TO TESTCan HbF be raised durably and safely without a transplant-style procedure?
WITHOUT CONDITIONING CHEMOTHERAPY PRECLINICAL HYPOTHESIS

Reach stem cells inside the body and switch fetal-globin genes back on, without myeloablative chemotherapy.

MATCHED EVIDENCE
FOUNDATION
Xu et al. in-vivo HBG promoter editing

LIMITXu et al. edited human cells in mice using a beta-thalassaemia model, not people with sickle cell disease. Related viral work used mobilisation and chemoselection, so it is not evidence of a chemo-free treatment.

WITH CONDITIONING CHEMOTHERAPY HUMAN EVIDENCE, LIMITED ACCESS

Edit collected stem cells to reactivate HbF, then reinfuse them after marrow-conditioning chemotherapy.

LIMITCasgevy has managed NHS access only for eligible patients in England. The Boston BCL11A pilot, risto-cel and CS-206 remain investigational. Each platform is distinct, the Boston pilot has no posted outcomes, and conditioning carries major risks.

NEXT EVIDENCE GATEHuman evidence for sustained HbF, safety and fewer crises without myeloablation or chemoselection.

GLOBAL / SUB-SAHARAN AFRICA/PAEDIATRIC STROKE PREVENTION

SPRINT follow-up reports no significant stroke or mortality difference by initial hydroxyurea assignment

The article abstract reports that recurrent stroke and all-cause mortality did not differ significantly by original fixed low- versus moderate-dose hydroxyurea assignment; events clustered in the first year. The abstract does not provide evaluable group sizes, event counts or effect estimates.

Limits and UK relevance

UK relevanceThis is not a UK treatment-access or guideline change. UK secondary stroke-prevention decisions require specialist guidance and cannot be inferred from this cohort alone.

LimitationsNo statistically significant difference is not proof of equivalent dose strategies. The full paper could not be independently inspected at publication, so group counts, crossover, follow-up completeness and safety detail are not confirmed from the abstract. The original trial dates from 2016 to 2020; this is a new long-term report, not a new trial.

Adds longer-term evidence for secondary stroke prevention in children with sickle cell anaemia in a resource-limited setting, while highlighting early risk after an initial stroke.

MEDIUM CONFIDENCE

Blood online-first article dated 26 October 2026 reports 4.9 years of pragmatic follow-up. Original SPRINT registry NCT02675790 first posted 5 February 2016, last updated 19 January 2022, completed 31 March 2020, with no registry results posted.

UNITED STATES / CITY OF HOPE/INVESTIGATIONAL TRANSPLANTATION

City of Hope haploidentical transplant pilot withdrawn before enrolment

The sponsor changed the study to withdrawn and says it closed to accrual because no participants accrued in its clinical-trial management system. The planned approach combined pre-transplant immunosuppression, myeloablative conditioning and haploidentical donor transplantation.

Limits and UK relevance

UK relevanceNo change to UK transplant commissioning, eligibility or available care.

LimitationsThe registry retains an estimated primary completion date of 25 January 2027 despite withdrawal; this is not an active milestone. No participants were treated, so no efficacy or safety inference is possible. This is one investigational protocol, not a withdrawal of haploidentical transplantation as a treatment category.

The proposed US phase 1 pathway will not generate the anticipated human treatment evidence from this registered study.
